What Is Orthopedic Therapy?

Orthopedic therapy treats the parts of you that carry load: bones, joints, muscles, tendons and ligaments. It is the care people come to after a knee replacement, after a rotator cuff repair, after a fall that never quite healed right, or after years of a shoulder that has slowly stopped doing what it used to. Your therapist starts by finding the actual source of the problem, which is often not where the pain is loudest, then builds a plan around that joint and the way you use it. Treatment blends hands on work with graded loading: joint mobilization, soft tissue work and stretching to restore the movement that stiffness took, then progressive strengthening so the joint can hold that movement under real demand. Dr. Kirsten Harper, one of our physical therapists, has a stated interest in orthopedic manual physical therapy, and that hands on approach runs through how we treat here. In Texas you can begin without a physician referral for up to 30 days of treatment.

Left alone, an orthopedic problem rarely stays still. A joint that hurts gets used less, the muscle around it gets weaker, and the load shifts to something that was never built to take it. That is how a hip problem quietly becomes a back problem. Orthopedic therapy interrupts that sequence and rebuilds in the other direction: range first, then strength, then the movement habits that protect the joint at work and at home. Most people notice a change within the first two to three weeks, and a typical course of care runs two to three visits a week for four to six weeks.

Recovering after orthopedic surgery

A knee or hip replacement, a rotator cuff repair, a fracture that has finally been cleared for loading. The surgery fixed the structure, but the joint around it is stiff, the muscle has shrunk, and your confidence has not caught up. Your therapist works to your surgeon's protocol while treating the whole limb rather than the incision, restoring range early and adding load as the tissue allows. What you gain is a joint you use normally again instead of one you protect all day.

An injury that never fully settled

The sprain, the strain or the fall was months ago and you told yourself it would sort itself out. Instead you adapted around it. Old injuries usually linger for an ordinary reason: range of motion never came back, or the muscle that guards the joint stayed switched off. Your therapist works out which it is, treats it directly with hands on work and targeted loading, and rebuilds the tolerance the joint lost. What you gain is a movement you stopped attempting, back on the list.

Ongoing joint pain from arthritis or tendinitis

Some mornings decide themselves before you are out of bed. Persistent joint pain responds better to graded movement and manual therapy than to rest, because an unloaded joint stiffens and weak muscle leaves more of the work to the joint surface itself. Your therapist finds the amount of loading your joint can genuinely tolerate today and grows it from there, alongside posture and body mechanics changes that lower the daily strain. What you gain is fewer flare days and more usable ones.

What to expect in a session.

Every visit is one on one with the same therapist, in a block of 30 to 60 minutes, so the plan carries forward instead of restarting each time.

Evaluation and finding the real source

Your therapist measures range of motion, tests strength around and above the painful joint, and watches you perform the movements that provoke symptoms. Your history, your imaging and any surgical protocol all go into the picture. By the end you know what is driving the problem, what the plan is, and roughly how long it should take in your case rather than a generic answer.

Hands on treatment and graded loading

Sessions usually open with manual therapy: joint mobilization, soft tissue work and stretching to give the joint back the range it needs. Then comes the loading, because range you cannot control is not yet useful. Exercises are chosen for the specific weakness found at your evaluation and progressed as you tolerate them, with your therapist adjusting on the spot when something feels off.

Movement retraining and a plan that holds

The last phase is about how you use the joint outside the clinic: how you stand, lift, climb stairs and sleep. Your therapist retrains the mechanics that keep loading the joint badly, then gives you a short home program you will realistically do. You leave knowing what to keep doing and what to watch for.

Good to know

Wear clothing that lets your therapist see and reach the joint, so shorts for a knee or a hip and a loose sleeve for a shoulder.
