What Is ASTYM?

ASTYM is a specific, brand name treatment system for soft tissue that has healed badly. A trained clinician runs a set of purpose built instruments along the length of the affected tendon, ligament or fascia, following a protocol that does not change based on how the day is going. The point is not to grind out a knot. The pressure and the direction are chosen to provoke a regenerative response, so the body clears out degenerated tissue and lays down healthy, organised fibres in its place. That is why ASTYM is never delivered on its own. Every pass with the instruments is followed in the same visit by stretching and by loaded strengthening, because new tissue organises along the lines of stress you put through it. Tendon pain that keeps coming back

Achilles, patellar, tennis or golfer's elbow, a gluteal tendon that aches on the stairs. You warmed up longer, dropped the volume, took a spell off entirely, and it lit up again within days of returning. Tendons irritated for months stop remodelling on their own, and rest alone does not restart that process. ASTYM gives the tissue a reason to rebuild, then loads it so the new fibres line up with the way you actually move. The aim is straightforward: you return to the activity that used to trigger it and it stays quiet.

Scar tissue after surgery or injury

The incision healed and the joint still will not give you that last stretch of range. Scar tissue is strong but disorganised, and it binds layers that are supposed to glide across one another. What to expect in a session.

An ASTYM visit follows the same shape every time, and that consistency is the point. Here is how the treatment runs once your therapist puts hands on the area.

Evaluate and map the tissue

Your therapist tests range, strength and the movements that reproduce your symptoms, then works along the whole chain to find where the tissue has thickened or bound down. What comes back rough under the instrument is often not where you feel the pain, which is why the treated area is usually larger than you expected walking in.

The instrument passes

A light lubricant goes on and the instruments are drawn along the tissue in the set sequence. You feel a gritty, scratchy sensation over restricted areas and almost nothing over healthy tissue. Pressure is adjusted to what you can tolerate while still being useful, and your therapist keeps checking in as the passes continue.

Stretch it, load it, take it home

Straight after the passes you stretch the treated tissue, then strengthen it under real load while the healing response is switched on. This is the half that makes it hold. You leave with a short home programme, usually a stretch and a loading exercise, to repeat between visits.

After your session

Mild soreness and some pink or red mottling over the treated area for a short spell afterwards is expected, not a complication. Keep using the area normally, because sitting still is the one thing that works against the treatment. Tell your therapist before you begin if you take blood thinners, bruise easily, or have a skin condition, an open wound or a recent fracture near the area.
